To be considered for a Homewood event as a vendor, please read the following information carefully and thoroughly before emailing for help.
The link below is a general intake form to receive an application to apply to be a Homewood event vendor when an prior to a Homewood event application going live (3-6 months prior to event date). This form DOES NOT guarantee you'll be an approved vendor, but rather a way for us to carry a database of possible vendors for the future.

Food Vendors:
If you plan to sell edible items, please note the following requirements:
- Submit a Certificate of Liability Insurance.
- Complete the Cook County Temporary Food Vendor Permit Application with all required documents.
- Receive approval from the Cook County Department of Public Health (CCDPH) before selling food items.

How to Submit Food Applications:
- Applications must be submitted to Shelli Walton of CCDPH at swalton@cookcountyhhs.org
- All applications must include all required paperwork for full confirmation of participation. Incomplete applications will not be approved.
- CCDPH will coordinate directly with vendors and the Village to ensure compliance.
Liquor Vendors:
Already Licensed Liquor Holders in Homewood:
As a business that holds a liquor license in Homewood, in order to sell alcohol off your premises at a special event, you are required to obtain a Village of Homewood approval letter as well as a State of Illinois Special Use Permit Liquor License. You are required to submit a copy of your liquor liability insurance specific to the event date and location with the Village of Homewood listed as an additional insured as well as your State of Illinois Special Use Permit to the Village prior to the start of the event. You also need to have an owner or employee who is BASSET certified on-site for the duration of the event.
There isn’t a Village of Homewood fee for this request. In order to process these requests more efficiently, there is a Liquor Licensee Special Event application with a cover sheet explaining the process. The application should be completed, printed, and signed by the owner (with an original signature) before returning it the Village of Homewood.

Not-For-Profits Obtaining A Liquor License:
If you're a not-for-profit and would like to sell alcohol at an event in Homewood, you are required to obtain a Village of Homewood Class 6 Liquor License, as well as a State of Illinois Special Event Retailer’s Liquor License (not-for-profit). You are required to submit a copy of your liquor liability insurance certificate specific to the event date/location with the Village of Homewood listed as an additional insured along with a copy of your State of Illinois Special Event Retailer’s Liquor License (not-for-profit). Below is the Village of Homewood Class 6 liquor license application which includes a $41 fee: $25 for the license and $16 for the background check. Please read the cover page on the application that explains the process. The codebook description for the Class 6 license is below:
Class 6. Special license. This license is a special permit for the sale of beer and wine only and may be granted from time to time to clubs, associations and lodges organized for objects and purposes other than the consumption of alcoholic liquor and organized for at least two years before the date of application for license. Special licenses may also permit the sale of mixed drinks upon the approval of the village liquor commissioner.
