Cook County Department of Public Health Mitigation Guidance

The United States, Illinois and Cook County are in the early stages of a large surge of COVID cases due to the Omicron variant. On Dec. 23, 2021, Cook County Department of Public Health issued Mitigation Order 2021-11 to protect the health of the public in suburban Cook County. CCDPH’s jurisdiction does not include Chicago, Evanston, Oak Park, Skokie or Stickney Township. Click on the links below for more information.

Effective January 3, 2022, all indoor settings where food or drink are served for on-premises consumption, and health and fitness centers are required by CCDPH Mitigation Order 2021-11 to check the vaccination status of their patrons, and only allow entry for fully-vaccinated individuals age 5 and older.   Additionally, employees of businesses are required to be vaccinated, or must provide a weekly  negative COVID-19 test.

Businesses are required to post signage that advises their patrons of this requirement at the public entrances of the business. Sample signs are available for business use here.

Businesses must also develop a written plan to document how they will enforce the order.  A plan template is available here.

Order 2021-11 maintains the indoor mask requirement for all people in indoor setting that was issued in Order 2021-10.  No provisions of the masking order have been altered.

All Indoor settings where food or drink are served for on-premises consumption, and health and fitness centers must comply with the vaccination requirements in Order 2021-11.
